Distribution:
When there is an exceptional circumstance and a child cannot access a safe car seat to facilitate a timely discharge, the following car seats may be available for distribution. This program is subject to availability.
Social Work may complete the following Car Seat Requisition Form from Child Safety Link to request a safe car seat. It is expected that all other avenues are exhausted first.
